This written claim must include all potential theories of liability, and include all of the claimants. It must state the relevant facts (i.e. when, where, who, and why) and it must be filed within 6-months from the date of the injury producing incident. Failure to file this statutorily mandated administrative claim within the 6-month designated time frame may bar a plaintiff from pursuing any further legal action through the courts.
